At The Heart & Mind Collective, we believe you shouldn’t have to piece that puzzle together alone. That’s why we are thrilled to formalise our partnership with the Central Coast Local Health District (CCLHD) Menopause Referral Service and the Royal North Shore Menopause Hub.

The CCLHD Menopause Referral Service is a clinician-led initiative specifically designed for women navigating severe or complex symptoms. The beauty of this service is its "whole-person" approach. Through this pathway, patients gain access to a powerhouse of expertise across various medical fields:
Gynaecology & Endocrinology: Addressing the core hormonal changes and reproductive health.
Rheumatology: Supporting bone density and managing joint health.
Nursing & Women’s Health Physiotherapy: Providing hands-on clinical care and pelvic floor support.
Dietetics: Navigating metabolic changes and nutrition tailored to hormonal health.
Social Work & Mental Health: Offering essential advocacy and emotional support during this life transition.
How to Access Specialised Support
This service was established to increase healthcare capacity and ensure that Central Coast women receive the specialised care they need without having to travel between disconnected providers.
The Referral Pathway: General practitioners, specialists, and nurse practitioners can refer eligible patients to the specialised menopause hub.
Expert Leadership: The service is led by expert Women’s Health Registered Nurses and Physiotherapists who provide a direct link to the specialised hub at Royal North Shore Hospital.
Integrated Care: At The Heart & Mind Collective, we work alongside these clinical specialists to ensure your mental and emotional health is prioritised just as much as your physical symptoms.
